Tá sé/sí as a mheabhair/meabhair = He/she is out of his/her mind
Tá sé/sí craiceáilte = He/she is crazy/'cracked'
Tá a chiall/ciall caillte aige/aici = He/she's lost his/her sense
Tá sé/sí ar buile = He/she's mad
Tá mearadh air/uirthi = He/she's insane

Italian:

"comprendonio" is a fake word, mispelling understanding for fun...

Norwegian: Du er gal = You are crazy
Å ha en skrue løs = to have a loose screw
Mangler noen øre (på en hel krone) - A few cents/pence missing (for a full dollar/pound). Use with care! This expression can also mean someone is retarded.

Polish
Nie ma wszystkich w domu = He/She doesn't have them all at home.
Zwariował/a = He/She is gone mad.
Odbiło mu/jej = It's jumped off from him/her.
(On/Ona) Ma bzika = He/She is off his/her rocker.
Brak mu/jej piątej klepki = He/She lacks the fifth stave.

Portuguese:
Tem um parafuso a menos = S/he lacks a screw
Não tem os parafusos todos = S/he doesn't have all screws
É passado da cabeça = S/he's strained from the head
You can replace 'head' with less appropriate words ;-)

in Brazil, you can also say:
Ele não regula bem = S/he doesn't regulate well"
Ele não regula bem da bola = Her/His ball doesn't regulate well

Romanian:
Eşti nebun (for masculine)= You are crazy (for males)
Eşti nebună (for feminine)= You are crazy (for females).
Nu eşti în toate minţile = You aren't in all your minds.
Eşti nebun(ă) de legat = You are so crazy you need to be tied.
Ai păsărele la cap/sub pălărie = You have little birds in the head/under the hat.
Eşti într-o ureche = You are in one ear.
Îţi lipseşte o doagă = You're missing a barrel hoop.
Eşti dus cu pluta = You're away on a raft.
Eşti dus de-acasă = You're away from home.

Russian:
�?ойти �? ума = to go crazy
Он �?ошёл/ Она �?ошла �? ума = He/She has gone crazy.
у теб�?/ него крыша поехала! = your/ his roof has slidden!
